Financial Policy
The patient’s portion of the bill is due at the time services are rendered unless other arrangements are made in advance. All professional services and materials are charged to the patient. The patient will ultimately be responsible for any bill incurred by Bay Family Eye Care regardless of insurance. Accounts 90 days old are subject to collection fees. There will be a service charge on all returned checks.
Payment from the patient’s insurance should be paid directly to Bay Family Eyecare LLC. The patient’s primary insurance will be billed on behalf of the patient. Benefits quoted to the patient are not a guarantee of payment by the patient’s insurance company. Final benefit determination can only be made when the claim is processed.
