What to do when you arrive for you appointment

IF YOU HAVE A NEW INSURANCE SINCE YOUR LAST APPOINTMENT, IT IS YOUR RESPONSIBILITY TO NOTIFY US NO LATER THAN 2 BUSINESS DAYS PRIOR TO YOUR APPOINTMENT.

If You come in for your appointment with a new insurance plan not previous provided you will be responsible to pay the full amount of your visit and wait for reimbursement.

Coming in to your appointment with a new insurance poses an unnecessary strain on the front desk and interferes with your appointment time. Every plan is different and unique, and it can take over an hour sometimes just to get an insurance representative on the phone. We cannot treat you effectively or timely unless this is done prior to your appointment. If you chose to same-day cancel because of this then you will be charged a non-refundable fee of $100 for a broken appointment.

  • We are now offering a wellness package in lieu of dilation to speed up your time in the office. The fee for the wellness package is $49 regular or $69 for diabetics. Routine eye exams are REQUIRED to do either dilation or wellness in order to be seen. Dilation is FREE under your insurance but, does add approximately 20-30 minutes to your examination time. The wellness package takes approximately 5 minutes. If you book an appointment and opt to not do either once you present in the office there is a $100 broken appointment fee.
